Research shows that saw palmetto extract has been used extensively in the treatment of lower urinary tract symptoms and improves prostate health. And although the results are mixed, studies have shown that saw palmetto is also effective in improving erectile dysfunction. An animal study, for instance, evaluated the erectile functions of rats and rabbits that received saw palmetto extract. Researchers found that saw palmetto caused a relaxant response within the erectile tissue of the animals, allowing for increased blood flow. (22, 23).
Before you conclude that brand-name Viagra is superior for that reason, though, consider how much more leeway sildenafil’s strength gives you and your doctor to dial in the best dosage for you. You’ve got more control over it, and of course, the best dosage of any prescription medication will be the smallest dosage that produces the desired effects. Smaller dosage means less cost to you and less risk of side effects. And sildenafil is already much cheaper than Viagra.
If you’re older, there’s probably a physical reason for your ED. But the causes can be in your head, too. Experts say stress, depression, low self-esteem, and performance anxiety can short-circuit the process that leads to an erection. These factors can also make the problem worse if your ED stems from a physical problem.
All PDE5i medication does not improve erections in normal men, only in those with difficulty in achieving or maintaining erections sufficient for sexual intercourse due to a true medical problem.
Sildenafil (Viagra) was the first oral phosphodiesterase type 5 (PDE5) inhibitor approved by the FDA in the United States for the treatment of erectile dysfunction (it is not approved for women). Sildenafil inhibits PDE5, which is an enzyme that destroys cGMP. By inhibiting the destruction of cGMP by PDE5, sildenafil allows cGMP to accumulate. The cGMP in turn prolongs relaxation of the smooth muscle of the corpora cavernosa. Relaxation of the corpora cavernosa smooth muscle allows blood to flow into the penis resulting in increased engorgement of the penis. In short, sildenafil increases blood flow into the penis and decreases blood flow out of the penis.
Vitamin D deficiency has been linked with rickets, cancer, cardiovascular disease, severe asthma in children and cognitive impairment in older adults. Causes include not ingesting enough of the vitamin over time, having limited exposure to sunlight, having dark skin, and obesity. Symptoms include bone pain and muscle weakness. Treatment for vitamin D deficiency involves obtaining more vitamin D through supplements, diet, or exposure to sunlight.

Even if you take the medicine, you still need physical and mental stimulation and desire to have an erection. If your first dose doesn’t help, call your doctor. Your doctor may want to change your tablet strength.
Erectile Dysfunction (ED), also known as impotence, is a type of male sexual dysfunction characterized by the inability to maintain an erection long enough and firm enough for sexual intercourse.
Patient can inject medications directly into the corpora cavernosa to help attain and maintain erections. Medications such as papaverine hydrochloride, phentolamine, and prostaglandin E1 (alprostadil) can be used alone or in combinations to attain erections. All of these medications are vasodilators and work by increasing blood flow into the penis. Prostaglandin E1 (Caverject, Edex) is easier to obtain; however, it is associated with penile pain in some individuals. The use of combinations of two or three of these medications can decrease the risk of having penile pain.
"It is always worth reducing obesity, improving nutrition and getting more exercise -- firstly, because health and well-being improve and overall cardiovascular risk and risk of diabetes will be reduced," said Wittert, a professor and director of Freemasons Foundation Centre for Men's Health at the University of Adelaide, in Australia.
The type of prescription medication that’s most commonly used in the treatment of erectile dysfunction is phosphodiesterase type 5 inhibitors (including sildenafil, vardenafil and tadalafil). They are used to relax and open the penile tissue, allowing for blood to enter the penis and cause an erection. Intracavernous (base of the penis) injections of vasoactive agents, such as prostaglandin E1, are also used for ED. (32)
Monitoring erections that occur during sleep (nocturnal penile tumescence) can help you and your doctor to understand if the erectile dysfunction is due to psychological or physical causes. The nocturnal penile tumescence test is a study to evaluate erections at night. Normally men have three to five erections per eight hours of sleep. The test can be performed at home or in a sleep lab. The most accurate way to perform the test involves a special device that is connected to two rings. The rings are placed around the penis, one at the tip of the penis and the other at the bottom (base) of the penis. The device records how many erections occur, how long they last, and how rigid they are. The test is limited in that it does not assess the ability to penetrate.
